RESUMO

El sarcoma de Ewing es una neoplasia de origen más frecuentemente óseo; otras localizaciones son excepcionales. En el caso de las presentaciones primarias intracraneales, resulta imprescindible descartar que se trate de un secundarismo así como también de otros tumores neuroectodérmicos que puedan requerir distintos abordajes diagnósticos y terapéuticos. Se presenta a una paciente de 14 años que consultó por ptosis palpebral de ojo izquierdo asociado a diplopía de 2 meses de evolución; los estudios por imágenes mostraron una lesión tumoral extraaxial situada a nivel de la cisterna interpeduncular. Se realizó la exéresis completa, con diagnóstico anatomopatológico de sarcoma de Ewing de ubicación mesencefálica


Ewing's sarcoma is a malignant neoplasm mainly occurring in the bone, with other locations being exceptional. In the case of primary intracranial presentations, it is essential to rule out metastatic lesions as well as other neuroectodermal tumors that may require different diagnostic and therapeutic approaches. We present a 14-year-old patient who consulted for upper eyelid ptosis of left eye associated with a 2-month history of diplopia, with imaging evidence of extra-axial tumor lesion, located at the level of the interpeduncular cistern. Complete excision was performed, with a pathological diagnosis of Ewing's sarcoma of midbrain location.

BACKGROUND: IgG4-related disease (IgG4 RD) is an immune-mediated fibro-inflammatory disorder, with tissue infiltration of IgG4+ plasma cells. It causes pseudotumors, tumors, and a wide spectrum of clinical manifestations. AIM: To report the clinical, laboratory, histopathological and treatment characteristics of a group of Chilean patients with IgG4 RD. MATERIAL AND METHODS: Review of medical records of 52 patients aged 18 to 76 years with IgG4 RD seen at six medical centers. RESULTS: Elevated IgG4 serum levels (> 135 mg/dl) were found in 18 of 44 (41%) patients. There was histological confirmation of the disease in 46 patients. The most common sites of involvement were lungs, eyes and kidneys. Eighteen (35%) patients had only one organ involved, 34 (65%) patients had two organs and 13 (25%) patients had three or more organs. The involvement of two organs was significantly more common in men (p < 0.05). In patients with only one organ involvement, the most frequent location was orbital and meningeal. All patients with kidney or lung disease had multiorgan involvement. All patients received corticosteroid therapy, 67% synthetic immunosuppressants, and 16% rituximab. CONCLUSIONS: ER-IgG4 can affect any tissue. Multiorgan involvement was more common in this series, with preference for lungs, eyes and kidneys. An excellent response to steroids is characteristic of the disease, but with a high relapse rate that requires additional immunosuppression.

La seudomicroangiopatía trombótica o síndrome de Moschcowitz es una manifestación infrecuente del déficit de vitamina B12. Se caracteriza por anemia hemolítica con características microangiopáticas, reticulocitos e índices hematimétricos normales o con ligera megaloblastosis, asociados a manifestaciones neurológicas. La vitamina B12 está presente en alimentos proteicos de origen animal. La lactancia materna es una fuente adecuada para los niños cuando los niveles maternos son normales. Se presenta a una paciente de 16 meses que se internó por anemia hemolítica con requerimiento transfusional, plaquetopenia, mal progreso pondoestatural y retraso neuromadurativo. Durante su internación se arribó al diagnóstico de seudomicroangiopatía trombótica secundaria a déficit de vitamina B12.


Pseudo-thrombotic microangiopathy, or Moschcowitz syndrome, is a rare manifestation of vitamin B12 deficiency. It is characterized by microangiopathic hemolytic anemia, reticulocytes, and hematimetric indices that can be normal or that might present a mild megaloblastosis, and which are associated with neurological manifestations. Vitamin B12 can be found in animal-based protein foods. Breastfeeding is an adequate source of this vitamin for children, when maternal serum levels are normal. The case of a 16-month-old infant is presented. She was admitted for hemolytic anemia with transfusion requirement, thrombocytopenia, failure to thrive and developmental delay. During her hospitalization, she was diagnosed with pseudothrombotic microangiopathy caused by vitamin B12 deficiency.

La enfermedad por arañazo de gato es producida por la bacteria Bartonella henselae, bacilo Gram-negativo transmitido desde gatos y perros a los seres humanos a través de la inoculación cutánea por rasguños o mordeduras. Tiene un amplio espectro clínico, desde una forma típica caracterizada por una linfadenopatía regional cercana al sitio de inoculación hasta formas atípicas con compromiso sistémico. Las formas óseas son infrecuentes; representan, aproximadamente, del 0,2 % al 0,5 % de los casos.Se presenta a una paciente de 3 años de edad, previamente sana, que se internó por síndrome febril prolongado asociado a dolor y limitación del movimiento a nivel de la columna vertebral cervical, con diagnóstico de enfermedad por arañazo de gato con afectación ósea múltiple


Cat-scratch disease is produced by the bacterium Bartonella henselae, a gram-negative bacillus transmitted through cutaneous inoculation by a cat or dog's scratch or bite. It has a wide clinical spectrum, from a typical picture characterized by a regional lymphadenopathy near to the inoculation site to atypical pictures with systemic compromise. Bone compromise is infrequent, it represents around 0.2-0.5 % of the cases.We present a 3-year-old previously healthy patient, who was admitted with prolonged fever related to pain and movement limitation of the cervical spine. Diagnosis of cat-scratch disease with multiple bone involvement was reached.

El schwannoma es un tumor primario, habitualmente, benigno, procedente de las células de Schwann, productoras de la vaina de mielina que recubre los nervios periféricos. Constituye menos del 10 % de los tumores intracraneales y es infrecuente en la edad pediátrica.Se presenta a un paciente de 6 años y 11 meses de edad, previamente sano, con antecedente de cefalea holocraneana intermitente asociado a proptosis y disminución de la agudeza visual del ojo izquierdo, epífora y estrabismo, con evidencia tomográfica de una masa retroocular. Se realizó la exéresis macroscópicamente completa, con diagnóstico anatomopatológico de schwannoma orbitario


Schwannoma is a usually benign primary tumor. It develops from the Schwann cells, which produce the myelin sheath that surrounds the peripheral nerves. It represents less than 10 % of the intracranial tumors, and it is infrequent in the pediatric age.We hereby present a 6-year-and-11-month-old previously healthy patient, with a history of intermittent generalized cephalea associated with proptosis and a diminished visual acuity of the left eye, epiphora and strabismus, with radiological evidence of retro-ocular mass. A macroscopically complete exeresis was performed, with an anatomopathological diagnosis of orbital schwannoma

Background: Certain associated and specific myositis antibodies are related to certain clinical phenotypes of dermatomyositis (DM), disease severity and the presence of cancer. Aim: To describe the clinical profile of Chilean patients with DM and their associated and specific myositis antibodies. Material and Methods: Review of medical records of 15 patients with DM aged 31 to 72 years. Their clinical characteristics, laboratory tests and complementary tests were reviewed. In serum samples from each patient the presence of 16 specific antibodies was analyzed by immunoblot technique (Myositis Profile Euroline Blot test kit). Results: Fourteen (93.3%) patients had skin manifestations, five (33.3%) had pulmonary involvement, two (13.3%) had an associated cancer and nine (60%) had specific antibodies associated with myositis. Conclusions: These patients with DM had a clinical profile similar to what has been described elsewhere. The profile of myositis specific antibodies was different from reports in other populations.

Background The mitral annulus (MA) is a crucial structure that is in constant motion throughout the cardiac cycle. The main purpose of this study was to determine if M-mode evaluation of the longitudinal motion of the MA could be useful to examine atrio-ventricular interactions. Methods Echocardiographic data obtained from 150 patients (mean age 56 ± 16; 82 males) from the University of Cincinnati College of Medicine was evaluated to examine if any relationship exists between MA motion and measures of atrio-ventricular interactions. Results Even though left atrial size, left ventricular (LV) mass index, LV ejection fraction (LVEF) and degree of LV diastolic dysfunction (LVDD) were significant echocardiographic variables affecting MA motion; LVEF and the degree of LVDD were the main determinants of MA excursion during systole (MAPSE) and after atrial contraction (MAa). Our results confirm the surrogate value of MAPSE with regards to LVEF and also show that the extent of MA excursion during systole is the main determinant of MAa. The effect of LV diastolic function applies more strongly to MAPSE than to MAa. However, the maximal MAa amplitude varies in accordance to the type of LVDD. Conclusions We have shown for the first time that M-mode interrogation of the MA longitudinal motion appears useful to assess atrio-ventricular interactions. Since LV systolic and diastolic functions are so closely related; additional studies are now required to examine how this longitudinal measure correlates with known circumferential rotational data obtained with other imaging modalities.

ABSTRACT Background: The dual potential to promote tolerance or inflammation when facing self-antigens makes dendritic cells (DCs) fundamental players in autoimmunity. There is an association between smoking and DCs maturation in patients with rheumatoid arthritis (RA). However, ethnicity is a key factor in autoimmune disorders. Aim: To evaluate phenotypic and functional alterations of DCs obtained from Chilean patients with RA as compared to healthy controls (HC). In second term, to compare the inflammatory behaviour of DCs between smoker and non-smoker patients. Material and Methods: Monocyte-derived DCs and T-cells were obtained from blood samples isolated from 30 HC and 32 RA-patients, 14 of which were currently smokers and 18 non-smokers. Several maturation surface markers were evaluated in DCs, including HLA-DR, CD40, CD80, CD83 and CD86. Furthermore, autologous co-cultures of DCs and T-cells were carried out and then T-cell proliferation, and expansion of Th1, Th17 and Tregs were analysed. Results: Compared with HC, RA-patients displayed increased HLA-DR expression in DCs, which was manifested mainly in patients with moderate-to- high disease activity scores (DAS28). Furthermore, RA-patients presented a stronger Th17-expansion and a correlation between DAS28 and Th1-expansion. Both effects were mainly observed in patients in remission or with a low DAS28. Moreover, smoker RA-patients displayed enhanced HLA-DR and CD83 expression in DCs as well as an exacerbated Th17-expansion and a correlation between DAS28 and Th1-expansion. Conclusions: These findings suggest that smoking enhances the inflammatory behaviour of DCs and the consequent Th1 and Th17-mediated response in patients with RA


Introducción: El potencial dual que poseen para promover tolerancia o inflamación ante antígenos propios, hace de las células dendríticas (CDs) actores fundamentales en el desarrollo de autoinmunidad. Existe una asociación entre fumar y la maduración de las CDs en pacientes con artritis reumatoide (AR). No obstante, la etnicidad es un factor clave a considerar en desórdenes autoinmunes. Objetivos: Comparar las alteraciones fenotípicas y funcionales de las CDs obtenidas desde pacientes Chilenos con AR y controles sanos (CS). Además, analizamos las diferencias en el comportamiento inflamatorio que existe entre las CDs obtenidas de pacientes fumadores y CS. Materiales y Métodos: Se obtuvieron CDs derivadas de monocitos y células T desde muestras de sangre aisladas de 30 CS y 32 pacientes con AR, 14 de los cuales eran fumadores y 18 no fumadores. Se evaluaron marcadores de maduración en la superficie de las CDs: HLA-DR, CD40, CD80, CD83 y CD86. Además, se realizaron co-cultivos autólogos de células T y CDs, analizando la proliferación de células T, y la expansión de células Th1, Th17 y Tregs. Resultados: En comparación con los CS, los pacientes AR mostraron un aumento de la expresión de HLA-DR en las CDs, principalmente en los individuos con DAS28 moderado-alto. Los pacientes con AR presentaron una mayor expansión de células Th17 y una correlación entre el DAS28 y la expansión de células Th1, ambos efectos manifestados principalmente en los individuos con un DAS28 bajo o en remisión. Además, los pacientes con AR fumadores mostraron un aumento en la expresión de HLA-DR y CD83 en las CDs y una expansión de células Th17 exacerbada así como una correlación entre el DAS28 y la expansión de células Th1. Conclusiones: Nuestros resultados sugieren que fumar favorece el comportamiento inflamatorio de las CDs y en consecuencia la inducción de respuestas mediadas por células Th1 y Th17 en los pacientes Chilenos con AR.

Abstract Background: Left ventricular hypertrophy (LVH) is an important risk factor for cardiovascular events, and its detection usually begins with an electrocardiogram (ECG). Objective: To evaluate the impact of complete left bundle branch block (CLBBB) in hypertensive patients in the diagnostic performance of LVH by ECG. Methods: A total of 2,240 hypertensive patients were studied. All of them were submitted to an ECG and an echocardiogram (ECHO). We evaluated the most frequently used electrocardiographic criteria for LVH diagnosis: Cornell voltage, Cornell voltage product, Sokolow-Lyon voltage, Sokolow-Lyon product, RaVL, RaVL+SV3, RV6/RV5 ratio, strain pattern, left atrial enlargement, and QT interval. LVH identification pattern was the left ventricular mass index (LVMI) obtained by ECHO in all participants. Results: Mean age was 11.3 years ± 58.7 years, 684 (30.5%) were male and 1,556 (69.5%) were female. In patients without CLBBB, ECG sensitivity to the presence of LVH varied between 7.6 and 40.9%, and specificity varied between 70.2% and 99.2%. In participants with CLBBB, sensitivity to LVH varied between 11.9 and 95.2%, and specificity between 6.6 and 96.6%. Among the criteria with the best performance for LVH with CLBBB, Sokolow-Lyon, for a voltage of ≥ 3,0mV, stood out with a sensitivity of 22.2% (CI 95% 15.8 - 30.8) and specificity of 88.3% (CI 95% 77.8 - 94.2). Conclusion: In hypertensive patients with CLBBB, the most often used criteria for the detection of LVH with ECG showed significant decrease in performance with regards to sensitivity and specificity. In this scenario, Sokolow-Lyon criteria with voltage ≥3,0mV presented the best performance.


Resumo Fundamento: A hipertrofia ventricular esquerda (HVE) é importante fator de risco para eventos cardiovasculares, e sua identificação se inicia, geralmente, pela realização do eletrocardiograma (ECG). Objetivo: Avaliar, em hipertensos, o impacto do bloqueio completo do ramo esquerdo (BCRE) no desempenho diagnóstico da HVE pelo ECG. Métodos: Foram estudados 2.240 pacientes hipertensos. Todos realizaram ECG e ecocardiograma (ECO). Foram avaliados os critérios eletrocardiográficos mais utilizados para o diagnóstico de HVE: Cornell voltagem, Cornell voltagem produto, Sokolow-Lyon voltagem, Sokolow-Lyon produto, RaVL, RaVL produto, RaVL+SV3, Relação RV6/RV5, padrão strain, aumento atrial esquerdo e o intervalo QT. O padrão de identificação da HVE foi o índice de massa do ventrículo esquerdo (IMVE) obtido pelo ECO em todos participantes. Resultados: A média de idade foi de 11,3 anos ± 58,7 anos, 684 (30,5%) homens e 1.556 (69,5%) mulheres. Nos participantes sem BCRE, a sensibilidade do ECG para a presença de HVE variou de 7,6 a 40,9%, e a especificidade de 70,2 a 99,2%. Nos participantes com BCRE, a sensibilidade para a HVE variou de 11,9 a 95,2%, e a especificidade de 6,6 a 96,6%. Dentre os critérios com melhor desempenho para HVE com BCRE, destacou-se o de Sokolow-Lyon para voltagem ≥ 3,0mV com sensibilidade de 22,2% (IC 95% 15,8 - 30,8) e especificidade de 88,3% (IC 95% 77,8 - 94,2). Conclusão: Nos hipertensos com BCRE, os critérios mais utilizados para detecção da HVE pelo ECG apresentaram diminuição significativa de desempenho da sensibilidade e especificidade. Relata-se um caso de uma paciente de 50 anos, dislipidêmica, tabagista e hipertensa, que se queixava de claudicação intermitente progressiva há seis anos (durante primeira consulta ao deambular 100 metros). Ao exame clínico inicial, apresentava-se hipertensa (PA: 150 x 70 mmHg) e baixa amplitude de pulso nas artérias femorais, já em uso de quatro classes diferentes de anti-hipertensivos. Durante investigação diagnóstica, detectou-se, na angioressonância, estenose de 75% do lumen da aorta, 5 cm abaixo das artérias renais. Foi submetida à aortoplastia com stent, com satisfatória evolução clínica e normalização dos níveis pressóricos. Dessa forma, com as características clínicas descritas e com o tratamento utilizado para esta paciente, ilustramos um caso de coarctação atípica da aorta abdominal infrarrenal, patologia rara e com mecanismo fisiopatológico pouco compreendido. Destaca-se a importância do diagnóstico precoce e do tratamento adequado dessa síndrome.


We report a case of a 50 years-old patient, with dyslipidemia, that smokes and has hypertension, who complained of progressive intermittent claudication six years ago (during the first visit when walking 100 meters). The initial clinical examination revealed hypertension (BP: 150 x 70 mmHg) and low pulse width in femoral arteries, already in use in four different classes of antihypertensive drugs. During diagnostic investigation was detected in magnetic resonance angiography, 75% stenosis of the aorta lumen 5 cm below the renal arteries. She underwent angioplasty with a stent, a good outcome and normal blood pressure. Thus, with the clinical characteristics and the treatment used for this patient, we illustrate a case of atypical coarctation of the aorta abdominal below-renal, rare disease with poorly understood pathophysiological mechanism. We emphasize the importance of early diagnosis and appropriate treatment of this syndrome.

A epidemia mundial de obesidade e o reconhecimento da hipertensão como um dos maiores fatores de morbidade e mortalidade cardiovascular nos levam a realizar esta revisão, que visa entender o envolvimento do tecido adiposo e seu impacto no sistema metabólico. O tecido adiposo funciona como um órgão endócrino produtor de múltiplos moduladores imunes conhecidos como adipocitocinas. A obesidade leva ao aumento da expressão dessas adipocitocinas pró-inflamatórias no organismo, gerando um estado inflamatório crônico. O desequilíbrio dessas adipocitocinas parece ser a chave, a resposta moduladora do sistema inflamatório que contribui para as doenças cardiovasculares e, concomitantemente, com a hipertensão arterial.


The global obesity epidemic and the recognition of hypertension as a major factor of cardiovascular morbidity and mortality lead us to carry out this review, which seeks to understand the involvement of adipose tissue and its impact on metabolic system. Adipose tissue works as an endocrine organ that produces multiple immune modulators known as adipocytokines. Obesity leads to increased expression of these pro-inflammatory adipocytokines in the body, creating achronic inflammatory state. Thisimbalance of these adipocytokines seems to be the key in modulating the inflammatory system that contributes to cardiovascular diseases and concomitantly to hypertension.

Fundamentos: cresce em importância a relação entre níveis glicêmicos aumentados, porém não diabéticos (tolerância à glicose diminuída - TGD), fator de risco cardiovascular e hipertrofia ventricular esquerda (HVE). Objetivo: avaliar a massa do ventrículo esquerdo (MVE) em pacientes hipertensos sob tratamento ambulatorial e com TGD. Materiais e métodos: foram avaliados 358 pacientes hipertensos (com média de idade 59 ± 11 anos), divididos em três grupos: grupo I (hipertensos); grupo II (hipertensos com TGD) e grupo III (hipertensos com diabetes tipo 2). Em todos os pacientes foi realizado o ecocardiograma e utilizada a fórmula de Devereux para o cálculo da massa do ventrículo esquerdo (MVE). Para o índice de MVE (IMVE) foram considerados valores normais de até 89 e 103 g/m² para mulheres e homens, respectivamente. Utilizou-se o teste estatístico ANOVA complementado pelo teste de Tukey e Bonferroni para detectar diferenças entre os grupos. Resultados: a média do IMVE foi de 110,2 ± 33,0; 117,6 ± 30,1 e 128,0 ± 46,8 g/m² nos grupos I, II e III, respectivamente, sendo que 65,1% (grupo I), 74,6% (grupo II) e 80,5% (grupo III) dos pacientes apresentaram aumento do IMVE. Não houve diferença estatística no aumento de IMVE entre os grupos I e II, mas foi significantemente maior no grupo III, quando comparado com o grupo I. Conclusões: o grupo dos pacientes hipertensos com TGD (grupo II) não apresentou aumento de MVE estatisticamente significante em relação aos não intolerantes (grupo I) e o grupo de pacientes hipertensos e diabéticos apresentou aumento significante da MVE em relação ao grupo de hipertensos (grupo I).


Background: It is growing in importance the correlation between elevated glycemic levels (impaired glucose tolerance) but no overt diabetes and the risk factor of cardiovascular disease and increased cardiac mass. Objective: We sought to determine the left ventricular mass (LVM) in hypertensive patients under treatment and with impaired glucose tolerance (IGT). Materials and methods: 358 patients were evaluated (mean age 59 ± 11 years) separated in three groups: group I (hypertension); group II (hypertension and IGT) and group III (hypertension and diabetes mellitus 2). Echocardiograms were performed in all patients and the left ventricular mass index (LVMI) was calculated using the Devereux's criteria and values greater than 110 and 134 g/m² for women and man, respectively, were considered for left ventricular hypertrophy. The statistic methods ANOVA together with Tukey and Bonferroni tests were executed to detect differences among groups. Results: 110,2 ± 33,0; 117,6 ± 31,0 and 128,0 ± 46,8 g/m² were the mean of LVMI for groups I, II and III, respectively. The increasing of LVM was 65,1% (group I), 74,6% (group II) and 80,5% (group III). There was not statistic difference between groups I and II in relation to increasing of LVMI, but it was significantly greater in group III. Conclusions: The increasing of LVMI was not observed in the impaired glucose tolerance hypertensive patients when compared with normoglycemic hypertensive patients and the increasing of LVMI was significant in diabetic hypertensive patients.

Sabe-se que alguns fatores podem interferir na medida correta da pressão arterial, sendo a distensão vesical uma delas. O objetivo foi avaliar o comportamento da pressão arterial de acordo com a sensação de distensão vesical, expressa por vontade discreta, moderada e intensa de urinar, e após micção, comparando-se os resultados com os níveis basais em três grupos de pacientes: normotensos (grupo 1), hipertensos não controlados (grupo 2) e hipertensos controlados (grupo 3). Foram avaliados 149 pacientes, com idade média de 42,5 anos ± 19,3 anos. Era verificada a pressão arterial em condições basais. Após ingesta de 1000 mL de água mineral em até 30 minutos aferia-se novamente a pressão arterial, quando, a partir de uma escala de pontos, os pacientes referiam vontade discreta, moderada e intensa de urinar, e cinco minutos após micção. Comparando-se as condições basais versus vontade discreta, moderada, intensa e pósmicção para pressões sistólica e diastólica, os valores de p mostraram-se significantes apenas entre condições basais versus vontade discreta no grupo 3; condições basais versus vontade moderada nos grupos 1 e 3; condições basais versus vontade intensa nos grupos 1 e 3; e nas condições basais versus pós-micção para pressão diastólica do grupo 3. Portanto, há correlação entre distensão vesical, avaliada subjetivamente, pelo menos a partir da vontade moderada de urinar nos grupos 1 e 3. No grupo 2 não houve associação.


Some factors are known to interfere with the correct measurement of blood pressure, bladder distension being one of them. The objective was to evaluate blood pressure variation according to the sensation of bladder distension, rated as a mild, moderate or strong desire to urinate, and after micturition, by comparing the results with baseline levels in three groups of patients: normotensive (group 1), non-controlled hypertensive (group 2), and controlled hypertensive individuals (group 3). 149 patients with a mean age of 42.5 ± 19.3 years were evaluated. Blood pressure was measured at baseline, after the intake of 1000 mL of mineral water in up to 30 minutes, when, using a point score, the patients reported mild, moderate, or strong wish to urinate; then, again, five minutes after micturition. Comparing systolic and diastolic blood pressure measurements at baseline and at mild, moderate, and strong desire, as well as post-micturition, the p values were significant only between baseline and mild desire in group 3; between baseline and moderate desire in groups 1 and 3; between baseline and strong desire in groups 1 and 3; and between baseline and postmicturition for diastolic pressure in group 3. Therefore there is a correlation between bladder distension, as assessed subjectively, and blood pressure measurement, at least as from a moderate wish to urinate in groups 1 and 3. No association was observed in group 2.

Objetivo: Examinar el posible rol de medicaciones en el desarrollo de daño Neuropsiquiátrico en pacientes con lupus eritematoso sistémico (LES). Materiales y métodos: Se incluyeron pacientes con LES (Criterio del ACR, mayor e igual 16 años al momento de ser enrolados en el estudio, menor e igual 5 años de duración de la enfermedad) de la cohorte LUMINA (Lupus in Minorities: Nature vs. Nurture). El tiempo a desarrollar daño Neuropsiquiátrico (psicosis o impedimento neurocognitivo, convulsiones, neuropatía perférica o craneal, accidente cerebro vascular, mielitis transversal) fue examinando mediante regresiones univariables y multivariables de Cox. Además se realizaron análisis de propensidad para determinar el posible rol protector de la hidroxiclorquina en el desarrollo de daño Neuropsiquiátrico en estos pacientes. Resultados: Se estudiaron 632 pacientes con LES. La edad, etnia caucásica, actividad de enfermedad a lo largo de la misma, diabetes, y conductas anormales relacionadas con la enfermedad estuvieron asociadas con un tiempo más corto para el desarrollo de daño Neuropsiquiátrico. Por otro lado la foto sensibilidad, la anemia, el fenómeno de Raynaud, y el uso de hidroxiclorquina y dosis medias de prednisona estuvieron asociados con un tiempo más prolongado. La dirección de la asociación fue la misma cuando se realizó el análisis de propensidad pero la hidroxicloroquina y de dosis media de prednisona en el desarrollo de daño Neuropsiquiátrico en paciente con LES.


Objective: To examine the possible role of medication in the occurrence of neuropsychiatric damage in patients with systemic lupus erythematosus (SLE). Material and methods: SLE patients from the LUMINA (Lupus in Minorities: Nature) cohort (ACR criteria, major and equal 16 years of age at enrollment and minor and equal 5 years of disease duration) were studied. Time to neuropsychiatric damage (defined according with the Systemic Lupus Collaborating Clinics (SCLICC) Damage Index (SDI): neurocognitive impairment or psychosis, seizures, cranial or peripheral neuropathy, stroke or surgical resection not due to malignancies and transverse myelitis) was examined by univariables and multivariable Cox regression analyses. Propensity score analyses were done to further determine the possible protective role of hydroxychloroquine in neuropsychiatric damage occurrence. Results: Six-hundred and thirty-behaviors were associated with a shorter time to the occurrence of neuropsychiatric damage whereas photosensitivity, anemia, Reynaud's phenomenon, hydroxychloroquine and a medium a medium dose of prednisone were associated with a longer time. Although the direction of the association remained the same by propensity score analyses (hydroxychloroquine), significance was no longer evident. Conclusion: Our data suggest a possible protective role of hydroxychloroquine and moderate doses of prednisone in the occurrence of neuropsychiatric damage in patients with SLE.
